Q. How can I obtain a Certificate of Insurance?

If you have already obtained a certificate online, go here. Otherwise, call 602.631.2570 or e-mail us.

Q. How do I file a First Report of Injury form?
A. Call 602.631.2029 or click here for a Form 101. The Form 101, once completed can be e-mailed as an attachment or faxed to SCF Premier to initiate the claim setup. Soon, policyholders will be able to file this report through our website for faster service.
